From the sellerThe whiskey was prescribed for B.C. Steinbrecker, by Dr. Thos. L. Dwyer, both of Chicago, IL, on August 23, 1933. Printed in red ink is: "This stub must be clearly and legibly written, and must not be detached from the book".
